Please note, you will be sent a welcome email when you make your initial appointment. In this email will be a link for you to complete your initial paperwork online- if you can please complete this before you come in for your appointment that would be greatly appreciated as you are helping us to reduce the amount of paper we use :)
If you for any reason you cannot complete the paperwork online, please arrive to your appointment 15mins early to complete the paperwork then.
